Intradural pituitary transposition. Both dural layers have been opened to expose the pituitary gland. The dura of the superior intercavernous sinus, tuberculum sella, and diaphragm has also been opened to allow for superior mobilization of the gland. Gentle dissection is used to separate the gland from the dura of the sellar floor and medial wall of the cavernous sinus. The anterior or medial wall of the cavernous sinus might be inadvertently opened while dissecting the gland off the dural walls, but it is not necessary to enter the cavernous sinus.
