Left cavernous sinus. The superior compartment is located above the horizontal cavernous ICA and the posterior compartment is behind the short ascending cavernous ICA. The interclinoidal ligament separates the superior compartment from the clinoidal triangle. The inferior compartment lies below the horizontal cavernous ICA and anterior genu of cavernous ICA. It contains the sympathetic or carotid nerve and the distal cavernous segment of the abducens nerve. Accessing the inferior compartment requires opening the anterior wall of the cavernous sinus below the anterior genu of the cavernous ICA. (Image courtesy of JC Fernandez-Miranda)
